Immunization & Health Documentation

Per Colorado Board of Health rule , all students living in college or university residence hall facilities in Colorado are required to provide evidence of protection from measles, mumps, and rubella (MMR) and meningococcal disease.  At Colorado College, this documentation as well as a TB Screening Form is submitted through the Admission applicant portal prior to arrival on campus. Please provide a scanned copy of each document, not a photo.  

Required Documentation

Immunizations 

  1. from a medical provider or practice* including documentation of:  
    1. Two valid doses of MMR vaccine (Measles, Mumps, and Rubella) with dose two at least 28 days after dose one.  
      1. Students may also submit documentation of laboratory confirmed positive titers for MMR or a certificate of exemption following Colorado’s process as outlined by CDHPE’s   
    2. Meningococcal (MenACWY or MCV4) vaccine on or after 16th birthday AND within the last 5 years (from residential move-in date)  
      1. Students may also submit a signed Meningococcal Disease Information and Vaccine Waiver found on page 2 of the Colorado Certificate of Immunization 

Please note, you may be asked to leave campus in the event of an outbreak, should you waive these immunizations.

Other accepted immunization records include: 

  • Official immunization record from CIIS (Colorado Immunization Information System)  
  • Official immunization record from another state, county, municipality, country, or regional public health authority 
  • Immunization record from a medical practice on official letterhead and/or signed by a medical provider 

Tuberculosis (TB) Screening 

  1. completed within the 6 months prior to arrival on campus. 
    1. Part 1 is completed by the student. If ANY question in Part 1 is answered with YES, Parts 2 and 3 must be completed by a health care provider. This PDF is fillable or can be printed and completed by hand.
